Group, Wisconsin's largest non-profit healthcare system is seeking
a Senior Care Physician to join our established team. You will be
responsible for working with PALTC patients in community living
facilities developing relationships while administering high
quality patient care. Position Details: Full-time or Part-time
options available with flexible schedules!Locations: 1 Physician
needed for the Fox Valley and 1 Physician needed for
Berlin/OshkoshThe team consists of 5 Physicians and 8 APC'sOn-Call
by phone onlyPatient census: 8-12 Support staff includes RNs and
schedulers who help to coordinate provider visits, communicate with
facilities, and triage phone calls.Senior Care Services:Visits
completed at patients location (Nursing Home or Assisted
Living)Acute visits, compliance visits, & discharge visits for
short and long-term residentsAdvance Care Planning & Goals of Care
discussionsMinor procedures at bedsideCommunity Overview:The Fox
